We're thrilled to have you on board. To help you get started smoothly, please take a moment to review the following:

Palliative Care Tele-Consult Request Form

1. Training Certificates

If you have certifications for the following CITI trainings GCP and HSPP(biomedical), please send them to our Coordinators. If not, please follow this guidance:

A) Go to the CITI Program Website

B) Register for an account using your URMC email. Will need to do HSPP (Biomedical) and GCP training, when asked about enrolling in courses it will be the new course (not the refresher) and greater than minimal risk

2. CareNAV Access

To access the platform for charting and recommending resources to patients and carepartners, please follow these steps:

A) Find your VPN access information. Connect to your VPN and navigate to What Is My IP Address

B) Email Suzzane and Bradeley to request a CareNAV login using your preferred email. Let Brad and Suz know if you have an office and if you use your VPN when in the office. Please remember to include your IPv4 and IPv6 numbers.

(If you do not have a VPN supplied by your institution, they have prepared a VPN for you, but will need to know more about your situation)

Instructional Videos for Navigating CareNAV

To assist you in navigating the platform effectively, please review these steps and watch all instructional videos to learn how to navigate our CareNAV resources:

1. Login

Link to HCP portal: cope.admin.carenav.org

- User ID is your email address
- I forgot my password link, use to create your own password

2. Cases Tab

- Shows all pt's/cg's in the intervention phase
- Can sort by status and by staff
- Likely won't need "Reports" button
- "Help" button in top right (Question mark) if you need to review how to do something w/in CareNav
- CG's from most recent to oldest
- Can use search bar based on client ID, phone number, email address, and name
- Statuses: whether someone is about to begin, etc.
- Eye: Case details page, basic profile page for each client
- Contact details, address, diagnoses, etc.

3. Messages Tab

- Can internally message with Caregivers and they can message with us

4. Users Tab

- Where all staff, clients are entered
- Can click "details" and change password
- You can also set client password

5. Progress Notes

- Add activities entry for new note
- Enter everything in .25 increments (15 minutes)
- Activity type: Choose who is writing the note
- Status: Completed or in Progress
- Facility type: Online or phone
- Comments: Note entry. Can drag box bigger in corner.
- When done, everything autosaves after clicking the "X"
- Will show up on client record afterwards

6. Action Plan

- After assessments, creating a care plan
- Draft, for working on something
- Active, CG will be able to see it

7. Documents

- Upload new document, material you want the caregiver to have and be uploaded
- Document becomes a part of their record
- Everything defaults to hidden unless you change it

8. Resource Library

- Sorting through resources for PT’s/CG’s, tracking what they’ve interacted with
- Matches what is seen from PT/CG portal, same list, same browse categories
- Can recommend an article or include on their action plan
- Can sort based on read, watch, attend, or listen, also based on language
- Go to Client Record (Notebook icon) to see if they’ve accessed it
